Health Coverage Statements (Forms 1095-B and 1095-C)
Under federal law, beginning with the 2024 tax year (reports issued in 2025), employers are no longer required to automatically send Forms 1095-B or 1095-C to individuals. Instead, these forms will be provided only upon request.
If you would like a copy of your Form 1095-B or 1095-C for your records or tax filing, you may request one at any time.
How to Request Your Form:
Please contact hrhelpdesk@equipenterprises.org to request a copy.
- Requests made by January 31 will be fulfilled by January 31 of the following year.
- Requests made after January 31 will be fulfilled within 30 days of receipt, or by January 31 of the following year, whichever is later.
This notice will remain available on our website through October 15 following the reporting year.
